INCIDENT AT SRP BATTERY STORAGE FACILITY IN CHANDLER

CHANDLER, Ariz. ' Chandler firefighters are working to contain a battery fire burning at a Salt River substation near 54th Street and Pecos Road.
During a press conference Thursday night, Chandler Fire Battalion Chief Keith Welch said firefighters had taken a defensive posture against the fire given the hazardous materials inside the facility.
Welch said robots were sent inside the building to open doors and ventilate the structure.
The Chandler Fire Department is working with businesses nearby and SRP to reduce the hazard potential at the site.
Officials said, as a precaution, businesses within roughly a quarter-mile area were asked to leave the area by 6 p.m. Thursday night.
An SRP spokesman said this is a 10-megawatt storage facility but that SRP has contingencies for situations like this and that customers should not be impacted by the temporary disruption at the substation.

An odor investigation in Pittsburgh's Mount Washington neighborhood prompted a HAZMAT response on Friday morning.
The incident began around 3:30 a.m. near the 1000 block of Grandview Avenue.
A Pittsburgh's Action News 4 photojournalist said the focus of the investigation seemed to be a sunken garage under an apartment building.
There were no reports of injuries.

The U.S. government's nuclear waste repository in New Mexico has major issues in fire training and firefighting vehicles, with its fleet in disrepair after years of neglect, according to an investigation by the U.S. Energy Department's Office of Inspector General.
The investigation was spurred by allegations regarding fire protection concerns at the repository, which is the backbone of a multibillion-dollar effort to clean up Cold War-era waste from past nuclear research and bomb making at national laboratories and defense sites across the U.S.
Investigators noted that the issues with the fire department training program went back to at least 2016. They pointed to an undeveloped training curriculum for the technical rescue program and claims by firefighters that their training needs weren't being met.
According to the inspector general's report, the issues persisted because the contractor that manages the repository inadequately addressed and closed recommendations from prior internal assessments that were aimed at fixing the deficiencies. The report also blamed inadequate oversight by Energy Department officials.

After almost a year of studying how "forever chemicals" touch nearly all aspects of life in Massachusetts, the PFAS Interagency Task Force released its final report Wednesday with recommendations that the state regulate PFAS chemicals as a class, restrict the sale of consumer products with intentionally-added PFAS and work to raise public awareness of the ubiquity of the problem.
Per- and polyfluoroalkyl substances (PFAS) are man-made chemicals that do not break down entirely in the environment, and exposure to their long-lasting presence has been linked to serious and negative health impacts like thyroid disease and kidney cancer.
PFAS chemicals are all around us; they are used in non-stick cookware, food packaging, children's products, carpets, leather goods, ski wax, firefighting foams and more, and they have leeched into drinking water supplies and the soil.

'They could tell that there was something in the air and they realized that it was unsafe for them to proceed,' Melanson said. The officers backed out and called the Windsor Volunteer Fire Department for assistance.
The fire department found an individual and took them to the hospital, according to Melanson. He believed the individual will recover.
Four officers who were overcome by the fumes were also taken to the hospital. Melanson added that the officers seemed to be doing well and believed the injuries were not life threatening.
'They do want to be checked just to make sure to identify what they were exposed to,' Melanson said.
As of 2 p.m., Melanson said officials have been unable to determine what caused the odor. The state Department of Energy and Environmental Protection has been conducting tests and 'has not been able to identify anything in particular that may have been causing it,' he said.
Melanson said he didn't believe the hazmat situation was intentional.
Before the fire department arrived, police officers started to ventilate the home by opening up the windows. The fire department also used ventilation systems to move the air out of the home.
Melanson said the issue was contained to the house and it is 'actually safe right now so we're turning it back over to the homeowners.'

CSB RELEASES SAFETY VIDEO ON PAPER MILL INCIDENT

THE US Chemical Safety Board (CSB) has released a new safety video about the 2020 fatal incident at the Evergreen Packaging paper mill in Canton, North Carolina, US.
On 21 September 2020, a fire erupted in a confined space when a heat gun fell into a bucket of flammable epoxy vinyl ester resin, killing two contract workers. Evergreen Packaging is a packaging and containers company, making paper and paperboard products. The incident occurred during a planned shutdown of its Canton paper mill, while maintenance and repair activities were ongoing throughout the facility.
CSB's new safety video includes an animation of the events leading up to the incident. While completing repairs in an upflow tower, Universal Blastco contractors began using a heat gun in the confined space. A fire erupted when the gun fell into a bucket of flammable resin and though the Blastco contractors were able to evacuate, two Rimcor contractors in the connected downflow tower were unable to escape.
The video explains that in its investigation, CSB identified four safety issues that led to the incident, including hot work and confined space safety. CSB Chair Katherine Lemos said that these factors are common safety issues in many major incidents investigated by CSB. The video included commentary from both Lemos and Lead Investigator Drew Sahli.

To prevent additional contamination by per- and polyfluoroalkyl substances (PFAS), Maine is set to become the first US state to prohibit the use of municipal and industrial sludge as fertilizer.
Application of PFAS-tainted sludge to agricultural fields in the state has caused contamination that polluted drinking-water wells and led some farmers to euthanize dairy cows and shut down organic vegetable production. Maine governor Janet T. Mills (D) is expected to sign legislation (LD 1911) in late April that would ban application of the material as fertilizer.
PFAS are a family of environmentally persistent synthetic compounds widely used in products such as firefighting foams and grease- and water-resistant coatings for paper. Some of these substances are toxic, including those found in tainted soils and wells in Maine.
The chemicals are carried in industrial and municipal wastewater to sewage treatment plants. They are not broken down by wastewater treatment, which primarily filters out organic solids before discharging water into rivers or lakes.
PFAS persist in those solids, even if the material is composted before it is spread on land. Crops can take up the substances from the soil, and water percolating through the soil can move PFAS into groundwater.

NORTON ' A Route 123 building housing a hardware store and a brewery was evacuated Wednesday afternoon after about 20 gallons of liquid chlorine spilled inside the hardware store.
One employee at Ace Hardware, 33 West Main St. (Route 123), was taken to Sturdy Memorial Hospital in Attleboro as a precaution, according to a fire official and employees.
Firefighters from Norton, Attleboro and North Attleboro responded to the store about 2:15 p.m. for a Tier 1 hazardous materials incident.
A Tier 1 emergency involves the release of a small amount of a hazardous material that can be handled by local emergency responders rather than the state regional hazmat team.
Fire Chief Shawn Simmons said the liquid chlorine was inside four or five containers when the accident occurred. Firefighters planned to 'neutralize' the chemical with baking powder in order to safely clean up the spill, Simmons said.

BEDFORD COUNTY, Va. ' Leaders say the Bedford County Wastewater Treatment Plant returned to its regular operations, following last week's hazmat incident.
Megan Pittman, director of administration for the Bedford Regional Water Authority, says crews were at the facility Friday night troubleshooting and cleaning the chemicals ' after 16 people were treated for non-life-threatening injuries.
Pittman says about 50 gallons of sodium bisulfite and sodium hypochlorite accidentally mixed together, creating a chlorine gas.
'Because there were two chemicals scheduled for delivery, the first chemical that was being delivered was thought to be the hypochlorite. However, it was not, so the hypochlorite and the sulfite ended up getting mixed into a tank,' said Pittman.
Extra protocols are being put in place at the facility, including more locks and labels on the tanks and better checklists for both employees and delivery drivers.

A chemical spill at BCP Ingredients in Verona on April 8 has been reported to national agencies, including the Environmental Protection Agency (EPA).
According to the EPA, BCP contacted the National Response Center (NRC) to report a release of an unknown quantity of ethylene oxide, a known carcinogen, at their facility in Verona following discovery of the leak on April 8. The following day, BCP Ingredients made a follow-up report to the NRC that included how the incident occurred, and and the amount of ethylene oxide that was released.
The NRC is a part of the federally established National Response System and staffed 24 hours a day by the U.S. Coast Guard. It is the designated federal point of contact for reporting all oil, chemical, radiological, biological and etiological discharges into the environment, anywhere in the United States.
The initial April 8 release report to the NRC by BCP reported that an unknown quantity of ethylene oxide was released from an off-loading pipeline on the company's property.
The April 9 report to the NRC by BCP Ingredients reported that 1,290 pounds of ethylene oxide were released while off-loading a pipeline. BCP reported that the cause was equipment failure at the plant from a loading pipe leading away from their rail-loading area.

(AP) ' A man charged with setting a fire that gutted a Northern California Home Depot, prompted hundreds to flee and filled the sky with smoke was trying to cover up a theft of tools, authorities said Tuesday.
Dyllin Jaycruz Gogue, 27, of San Jose, was arrested last Friday.
He appeared in court Tuesday and was given a public defender and a June 1 court date to enter a plea. He was being held without bail.
Gogue is accused of setting a fire that began in the lumber section of the Home Depot in South San Jose on April 9. He was trying to create a distraction while making off with a cart full of tools but drove off without taking them, prosecutors said.
The five-alarm fire destroyed the store, causing an estimated $17 million in damage to the store contents. The fire, which took about 100 firefighters six hours to control, sent up a huge black plume of smoke. People in nearby homes and businesses were told to stay inside out of concerns that the smoke might contain toxic chemicals.
The fire burned near hundreds of homes and was so hot that it was seen by satellites, San Jose Mayor Sam Liccardo said at a news conference.

Panaji: Given the large number of vessels transiting the Indian Ocean Region with oil and hazardous chemicals, the Indian Coast Guard on Wednesday urged friendly nations to consider a regional oil spill contingency plan along the lines of India's National Oil Spill Disaster Contingency Plan (NOS-DCP).
Director general of the Indian Coast Guard, V S Pathania said that any oil spill in Indian waters or neighbouring countries will affect all the nations.
'If there is an oil spill in a neighbouring country, it will spill onto you also because of shared waters and shared borders. That is why we are talking about a regional contingency plan and it is important to have it too,' said Pathania.

Recent years have seen significantly increased awareness of the need to improve the management of health and safety in academic chemical laboratories and, in particular, to reduce the potential for incidents posing a significant risk of fatality or serious injury. Studies have reported up to 30% of scientists working in academic chemical laboratories as having witnessed an injury at work severe enough to warrant attention from a medical professional. This paper is concerned with approaches to understanding and learning about why the individuals involved in incidents in chemical laboratories may have acted as they did. Drawing on two significant incidents as example and for context, the paper discusses seven themes that are typically of concern to those professionally engaged in the study and investigation of the Human Factors contributions to incidents: the difference between intentional and unintentional human failures; the concept of local rationality; four 'hard truths‰!
?? of human performance; the difference between 'work-as-done' and 'work-as-imagined'; different forms of complacency; consideration of how people might have benefited by acting the way they did; and the awareness and perception of risk and the difference between direct and indirect risk awareness. The paper includes recommendations for how different stakeholders, including academic institutions investing in new or modified chemical laboratories; administrators; environment, safety, and health (ESH) professionals; manufacturers and suppliers of chemicals and lab equipment; principal investigators; and graduate students could benefit from reflection on the seven themes. TRAIN COLLIDES WITH FUEL TRUCK, CAUSING SPILL, EVACUATIONS IN NA

NASHVILLE, Tennessee (WSMV) -- Nashville Fire responded to a crash involving a train and a fuel truck on Tuesday morning.
According to NFD, a train struck a fuel truck at the railroad crossing on 51st Avenue North around 8:30 a.m. Nashville Fire said the train was traveling at a low rate of speed but caused the fuel truck to spill its contents, nearly 5,000 gallons of fuel.
No fire was reported during the spill, and there were no injuries stemming from the incident.
'The tanker that was pierced we had to offload the remaining product from that tanker to another trailer and that is a very specialized process that our hazmat crews go through training for,' Nashville Fire Spokesperson Kendra Loney said. 'So, we are very grateful to have our crews trained and those techniques here.'

DAVIESS CO., Ind. (WEHT) ' One man has been pronounced dead after a 'hazardous materials incident' left him and another with serious injuries earlier this month.
Washington Indiana Police Detective Barry Hudson confirmed that on the morning of April 1 around 8:30, dispatch was contacted by an Eagle Railcar Services employee saying a man was down on the property. Officials said the call quickly changed to an incident involving two men down on the property.
According to Detective Hudson, life-saving measures were performed on both men by first responders on the scene before they were transferred to a hospital facility in Evansville. Family of James Ware, one of the men injured in the incident, confirmed with WEHT that he passed away on Easter.
The Indiana Occupational Safety and Health Administration says it is conducting a 'safety compliance inspection' that could take several months.

RAPID CITY, S.D. ' The large fire at Black Hawk business last week showed just how dangerous a fire like that can be not only to firefighters but also to the general public.
During the fire, large amounts of smoke filled with carcinogens and other contaminants became a point of contention among firefighters and the publics.
In fact, firefighters often come across hazardous materials when on the scene of a fire, which can be anything, according to Rapid City Fire officials.
'Any sort of chemical spill, gas release, unknown chemicals, anything like that,' said Captain Trapper Lappe, the Station Six Captain with the Rapid City Fire Department.
Many of the fire stations within RCFD specialize in something. Whether it's the department's Water Rescue Team at Station Three or the Hazardous Materials Team at Station Six.

Firefighters battled a major blaze for more than two hours at a chemical storage facility in Sydney's west this morning, with the flames spreading to adjoining properties and covering the city in a blanket of thick black smoke.
About 130 firefighters in more than 25 trucks rushed to the scene at St Marys in a bid to contain the fire.
A 39-year-old worker was treated for serious burns to multiple areas of his body before being airlifted to Royal North Shore Hospital in a serious condition.
LPG cylinders and aerosol containers exploded in the Cleanaway Liquid Waste Facility next door, complicating efforts to douse the flames.
One witness said the explosions sounded like "popcorn in the microwave", with a "bunch of aerosol cans just popping off everywhere".

PLAQUEMINE, La. (AP) ' The Louisiana Department of Environmental Quality and Olin Corp. were monitoring the air Tuesday morning at and around a plant where a fire and leak sent a huge cloud of chlorine out of a chemical plant near Baton Rouge.
No injuries have been reported, said a statement emailed to The Associated Press on Tuesday for Olin Corp.
'Monitoring indicates there is currently no risk of offsite exposure; however, we will continue to monitor the entire area out of an abundance of caution,' the company said.
State regulators said a compressor caught fire and leaked liquid chlorine, The Advocate reported.
Olin would not comment on whether a compressor caught fire. 'Once all work is safely completed, we will conduct a thorough analysis to identify the cause of the event,' its statement said.

Tuesday afternoon, the fire at the Nutrien Ag Solutions plant prompted evacuations for part of Leoti.
Update from AP: Officials say a fire at a fertilizer company in western Kansas has prompted evacuations Tuesday afternoon because hazardous materials were involved. Wichita County Clerk Lynda Goodrich says the Ford County Regional Hazardous Materials Team deployed to Leoti for the fire Tuesday afternoon and was fighting the blaze and removing hazardous material.
The fire began around 3:30 p.m. at a Nutrien Ag Solutions plant in Leoti. A company spokeswoman says no injuries were immediately reported. Wichita County officials requested people to avoid the area. The number of people evacuated isn't clear.
The Wichita County Sheriff's Office confirmed a chemical fire prompted evacuations in the northeast part of Leoti. Due to what the city described as 'a hazmat incident in the City of Leoti,' the city asked people to 'stay clear north of the railroad tracks.'

Safety evaluation of chemical reactions is very significant before industrial applications are conducted. A lot of thermal hazards are generated in the synthesis of 3,4-bis(4-nitrofurazan-3-yl)furoxan (DNTF) because of the strong exothermicity of the oxidation reaction, unstable hydrogen peroxide, and interactions between the materials. In this article, systematic parameters regarding the safe synthesis of DNTF were studied, including the preparation stage of reaction solution and the oxidation reaction step. A Calvet microcalorimeter and an RC1mx reaction calorimeter were employed for process safety research. Differential scanning calorimetry and adiabatic accelerating rate calorimetry were used to investigate the thermal stabilities of the materials and their thermal hazards. After screening of the thermal stabilities of the materials in the two stages, hazardous materials were identified, and their thermal safety parameters were obtained. Finally, two evaluation methods w!
ere applied to evaluate the hazard levels of the preparation process and the oxidation reaction process in the synthesis of DNTF on the basis of thermal safety parameters. This study provides critical safety technical guidance for the DNTF synthesis process and safety design.
