I am collecting detailed case study information on laboratory and pilot
plant incidents resulting from uncontrolled chemical reactivity hazards.
For example, a fire caused by the inadvertant contact of water on 
Sodium, or
a fire caused by the generation of Silane by mixing a Silicon compound 
with
acid.
 
If you have details of such incidents and phots of the resulting damage 
-
and can share them with me - I will certainly appreciate it.  I will 
protect
the identity of the incident, as appropriate.
 
This work is NON-COMMERCIAL and is being done on a pro bono basis for 
the
OSHA Alliance on Chemical Reactivity.  The end product of the project 
will
be a workshop on identifying and controlling Chemical Reactivity Hazards 
in
the Laboratory and Pilot Plant.  The workshop is currently scheduled for 
its
first presentation at the August meeting of the ACS in Boston.
