Suzanne,
I include the following section in the performance evaluations of my
laboratory staff. The process of evaluation includes a meeting with each
employee to discuss the evaluation results. This provides an excellent
opportunity to discuss any safety-related problems with that employee. 
;I encourage you to refrain from thinking of your evaluations as
"punishment." They serve an important function to protect your
employees from exposure to hazardous materials and unsafe work practices.
