Thanks to those who posted the list of violations or directed me to the bottom of the email for the reference.  I can say I learned something today about looking all the way through an email!).

 

With this list of violations, I am left to wonder just what they did have for a safety program in labs.  On the brighter side is that the University seems to be taking some action.
