You are correct. The Three Mile Nuclear Plant melt down disaster is an example of alarm versus interlock problems. Moreover, the three-mile scenario included a serious interlock initiation that was overridden by the Senior Reactor Operator (Shift Supervisor) on shift.
The same scenario repeated itself I years later at the Christal River Plant and operations did not override the emergency automatic functions. This resulted in a very minor inconvenience and restart later.
