Subject: Documenting Planning and Exposure Evaluation

We are conducting a review of our CHP and have identified the  
"planning" portion of our current CHP as being most problematic from  
both our and the labs' point of view. I wonder if anyone has developed  
a template that provides guidance to labs in addressing CHP planning  
needs such as conducting exposure evaluations, establishing criteria  
for implementing control measures, and identifying prior approval  
situations?

I'd be interested in gathering inspiration from others efforts in this  
regard.
