All,  There was an error in one of my posts:

Bill Reynolds from the Yale School of Drama informs me that the student death that brought me there as a consultant was the first and only death as far as he knows and he's been there since the 1970s!  I thought it was the second because when I was walking through the facility with EH&S, the fire marshal, the general counsel, and other people, I was referring to this fatal accident as the first in the department.  One of those people on the tour took me aside and told me it was acually the second death.  So from that time on, I thought it was the second. 
   
But I've done a lot of researching for this other phantom death and found nothing.   This is something I should have done before I accepted the statement of someone I can't even remember for sure now who!   My bad.
