We train about 25,000 people a year through our Tony Mazzocchi Center for Health, Safety and Environmental Education, in classes ranging from 2 hours to 2 weeks. We use an evaluation sheet at the end of the last session, but we get much better results when we are able to go back to the participants a couple of months later and ask what changed in their workplaces as a result of the training. We can only do that for a fraction of the trainings, but it works well.

I am not in academia, but that is basically the format we have been using for some time. If your interest is how the training is perceived, there would seem to be little possible variation, unless you want to get very specific about seating, sound quality, or other discrete presentation elements.  I’ll send Ralph an MS Word version of it in a direct email on Thursday.

The one NYCOSH uses just has 4 questions.   The first two ask you to grade the training on a scale of  poor, satisfactory, good or excellent on how you liked it and how well it met your specific needs for safety information.  Third asks for how you think the sessions could be improved, and fourth I think for any other comments.

I did a google search for such a survey, but most of the results concerned how 
training is delivered rather than the results from the participants' points of 
view. There is the approach of assessing safety climate, which could be 
considered the product of the training program, but I'm primarily interested in 
how the institutional safety training program itself is perceived at this point. 
It appears that I'll have to develop one myself.
