Grapefruit juice, with a likely culprit being bergamot and metabolites, is an inhibitor of cytochrome p450 mixed-function oxidase 3A4 (CYP3A4), one member of a very large family of enzymes expressed in a variety of tissues, but highly expressed in the liver, and involved in classical phase I metabolism of foods, xenobiotics, etc.
While there are a variety of foods, pharmaceuticals and environmental exposures that can modulate expression and activity of CYPs (which makes first year Toxicology classes quite interesting), one concern specifically with modulating CYP3A4 activity is that this single enzyme is reportedly involved in the metabolism of >40% of over the counter pharmaceuticals, and that there is no natural close ortholog of this enzyme in rodents.
