On September 30, 2018, a fire and explosion occurred after hours in a laboratory in PNNL's Physical Sciences Laboratory building. No one was hurt. After the event, an Accident Investigation Team worked to determine the causes. While the team was unable to conclusively determine the direct cause, this Lessons Learned article outlines what was learned from the event and the Team's recommendations to avoid recurrence.

This article is intended as a supplement to a video that extensively details what occurred, the experimental setup, damage to the fume hood, potential causes, and some of the lessons learned. HYDERABAD: After two major fires in chemical factories in the city, the disaster response and fire services department said there is a dire need to regulate the increasing number of chemical storage units and factories that have been set up on the outskirts of the city.
‰??Based on the risk factor, we divide the industries into three categories ‰?? low risk, medium risk and high risk. Based on this classification, the industries and factories are given suggestions on what kind of fire safety equipment they should use. Most of the reputed chemical factories follow the fire norms and conduct proper fire audits. The main problem is the mushrooming of small chemical factories,‰?? said V Papaiah, regional fire officer.

Three people were injured at a wedding after a compressed air cylinder spun out of control as guests were on the dance floor.

Garfield police received calls around 11:00 p.m. reporting that several people had been injured at the Venetian Catering Hall.

When police arrived they located the injured party guests inside one of the ballrooms.

Garfield Fire/HazMat, Hackensack University Medical Center EMS/ALS also responded to the scene, according to police.

FORT LAUDERDALE, FLA. (WSVN) - Crews have capped a gas leak in Fort Lauderdale after a construction crew accidentally hit a line.

Fort Lauderdale Fire Rescue crews responded to the scene at Northeast First Avenue and Fifth Street, at around 10:30 a.m., Saturday.

According to fire officials, a construction crew accidentally hit a gas line.

According to officials, two workers who were unloading 200 litres of chemical solvents to the storage shed of the Niramal Chemical factory triggered the fire due to a spark. The workers escaped with minor burns.

Firefighters are dealing with a hazmat situation after a fire at Louisville's Ford Assembly Plant.

A fire was reported on the roof around 11 a.m. Friday at the plant on Fern Valley Road.

Jefferson County Fire spokesperson Jordan Yuodis said the fire is out but crews are dealing with a Level 1 hazmat situation because the building has chemical hoppers on the roof. A Level 1 hazmat situation is for notification purposes only and does not pose a threat to the community or plant workers.

SAN ANTONIO ‰?? San Antonio firefighters were called to the scene of a small chemical spill in the 6800 block of Fairgrounds Parkway at noon Friday.

According to SAFD, crews arrived on the scene and discovered that a cleaning products business had a spill of less than 20 gallons of some type of acid that is used in their cleaning products.

When it came in contact with the concrete, it had a reaction that caused fumes to emit.

Everyone was evacuated out of the building in an abundance of caution.

DURBAN ‰?? A chemical fire in the industrial basin of Durban late on Friday afternoon left residents coughing and battling to breathe, according to community members.

The fire occurred at Hosaf, a fibre plant based in Mobeni, south of Durban, that manufactures polyethylene terephthalate resin and polyester fibre.

Rescue Care‰??s Garrith Jamieson said paramedics were called to the factory fire at about 4pm. ‰??Fortunately no injuries were sustained, however the fire department and all necessary role players were in attendance.‰??

By 6.30pm, residents from the Bluff and Umbilo areas had reported "a strong plastic smell", he said, as a strong wind carried the fumes over a wide area.

A Reddit user uploaded a dramatic recording of a Lime e-bike caught on fire at the UW Wednesday night.

PHILADELPHIA (KYW Newsradio) ‰?? Fire officials say they've disposed of about 50% of a dangerous chemical at Philadelphia Energy Solutions refinery, which was shut down by an explosive fire in June. However, they're still not declaring the scene under control.

‰??I'm pleased to say that we've had a very good week,‰?? said Philadelphia Fire Commissioner Adam Thiel.

He says the job of neutralizing Hydroflouric Acid at the site, by adding a base to it, is nearly halfway done.

Thiel declined to predict how much longer it would take to complete the job.

RIDGELAND, S.C. (WSAV) ‰?? Officials now say there is no threat to the air quality in Beaufort County from the massive debris pile fire just across the border in Ridgeland.

But the Environmental Protection Agency (EPA) admits they told businesses in the area to evacuate Thursday after a toxic chemical was found in the air.

That call prompted lawmakers from around the area on Friday to visit and see the problem for themselves.

‰??What I am being told by DHEC (Department of Health and Environmental Control) and EPA is past tests and current tests show no danger to the watershed. I‰??d like to have that verified, I‰??d like to see more tests done,‰?? said State Senator Tom Davis.
