You've got the more established graduate student led programs that I'm aware of.

To my knowledge the list below is not graduate student led, but may be of interest for historical context or comparison with similar goals.

I am a graduate student at the University of Connecticut and have recently changed my thesis topic to safety research. As one of my projects, I have been working with Kali Miller to compile a history and list of graduate student laboratory safety teams (LSTs).
