Koby,
I am a big fan of ENCON 160 goggles. They fit most of your criteria (no pun intended). They are available from Fisher Scientific and if you have a contract with them, you may be able to get a price break from your representative for a large order..
Full disclosure: I have no business relationship with ENCON and I am an occasional purchaser from Fisher Scientific.

Hello Everyone-
We're requiring our students to buy their own safety goggles next year because of the pandemic. I'm wondering if anyone has recommendations for goggles that are comfortable, do not fog up, and fit well even over eyeglasses. We'll be using
these in chem, bio, and environmental science classes. I teach at a private high school.
Also, any recommendations for safety glasses for our physics classes?
Thank you,
Koby
