Ian McCallum of Forgotten Weapons just posted an interesting video in
response to a recent video that showed the catastrophic failure of a .50
caliber rifle at the range. You may not be interested in firearms, but
Ian's video is an excellent study in their failure modes. He also
discusses engineering controls and the importance of paying attention
when something unexpected happens.
