I am not sure you all saw this:
I am not seeing the weekly "safety accidents" summary anymore. Is that gone?
Anyway, being Brazilian, this does not surprise me. Unfortunately, "accidents" like this and many others related to safety happen way too often.
Another one that I can't for the life of me find any English communication on (and I know you probably don't know Portuguese but in case you do: https://g1.globo.com/jornal-nacional/noticia/2022/10/05/vazamento-de-gas-toxico-mata-mulher-e-apavora-moradores-na-cidade-de-pontal-sp.ghtml) happened last year when a toxic gas release on a neighborhood killed a woman, left 100s hospitalized and many, many animals died. Despite the existence of a chemical a block away from the incident, "nothing wrong" was found.
