This is not a surprise to anyone who has followed EM's various enterprises and his rants reflecting his personal philosophy. He is a "full steam ahead and the hell with the consequences" person.
His enterprises, including Tesla, reflect this.
Sad, for a person who inspires such cool engineering.

Personally, this is the most vexing safety culture failure I’ve heard of since the disclosure of the injury rates at Amazon’s warehouse operations.
