New details have come out surrounding the Arizona Public Service (APS) battery failure and corresponding explosion that left eight firefighters and one police officer hospitalized in Surprise, Arizona in April of 2019.

All of the new details regarding the incident are put forth in two reports, one released by APS, the other by the Underwriters Laboratory Firefighter Safety Research Institute.

According to the APS report, developed by DNV GL Energy Insights, ‰??The suspected fire was actually an extensive cascading thermal runaway event, initiated by an internal cell failure within one battery cell in the BESS: cell pair 7, module 2, rack 15.‰?? The cascading thermal runaway is believed to have been caused by an internal cell defect, specifically abnormal Lithium metal deposition and dendritic growth within the cell.

And while the system‰??s clean agent fire suppression system began operating to contain the event, the system is designed to extinguish developing fires in ordinary combustibles, rendering it entirely ineffective against cascading thermal runaway. From there, the event spread through every cell and module in rack 15 of the system, via heat transfer, as the system did not have adequate thermal barrier protections between battery cells. These thermal barriers could have significantly deterred the spread.

VILLA HILLS, Ky. (WKRC) - Thousands of people in Northern Kentucky are concerned that a potentially dangerous chemical could soon be stored in their neighborhood.

TransMontaigne hopes to store styrene at the company‰??s terminal on River Road in Villa Hills.

Styrene is a chemical used to make latex, synthetic rubber and polystyrene resins. Those resins are often used to make plastic packaging.

An online petition calling for the Villa Hills Board of Adjustments to vote against TransMontaigne‰??s request has garnered more than 5,000 signatures. ‰??Stop Styrene‰?? signs have also been popping up along KY-8.

Craig Bohman, Villa Hills City Administrator, pointed out that TransMontaigne‰??s application is for the storage of styrene and not the production of the chemical.

TEMPE, AZ ‰?? Union Pacific apologized Thursday for the disruption it caused after one of its trains that was hauling lumber, chemicals, and other materials derailed, and subsequently caught fire, while crossing a bridge over Tempe Town Lake the day prior, and said it intended to repair the bridge and to eventually resume its operations.

Valley Metro was also able to resume light rail operations on Thursday afternoon. The light rail does not use the same bridge nor rail that the Union Pacific train was on. That line is primarily used for cargo trains.

Crews were also able to successfully contain the chemical leak from one of the railcars that fell into a dry bed when part of the bridge collapsed, said Tempe Fire Chief Greg Ruiz during a Thursday afternoon press conference.

That tank was leaking a chemical called cyclohexane, which is a flammable chemical sometimes used to make nylon or used as a paint thinner. Chief Ruiz said Thursday that an estimated 500 gallons had leaked out of the railcar, which have the capability to hold between 25,000 - 35,000 gallons.

US CHEMICAL SAFETY BOARD PRAISES AIRGAS FOR ITS RESPONSE TO 2016 FATALITY

The US Chemical Safety and Hazard Investigation Board (CSB) issued two significant reports in late July. One praises voluntary actions of Airgas after a deadly incident at a nitrous oxide facility. The second is a factual update to the ongoing investigation of an incident at an Aghorn Operating facility in Texas in which a worker and his wife died from exposure to toxic hydrogen sulfide gas.

The CSB is an independent, nonregulatory federal agency that investigates chemical accidents.

In a Safety Spotlight report, the CSB lauds Airgas, now a subsidiary of Air Liquide, for rapidly developing and implementing a robust process safety management program (PSM) in response to an explosion in 2016 that killed one worker.

CSB stresses that the effort was voluntary, as nitrous oxide handlers are not required by US federal law to develop a PSM program. The incident occurred during the loading of a trailer truck at an Airgas facility in Cantonment, Florida. In the transfer, a pump heated the nitrous oxide above its safe operating limits, causing decomposition and the explosion.

According to the CSB, after the incident, the company began to review its safety programs for its nitrous oxide production facilities, trucking fleet, and cylinder-filling operations. The review resulted in several reforms, such as better audit practices and inherently safer designs.

DEADLY INDIAN INCIDENTS HIGHLIGHT RELAXED REGULATIONS

A spate of chemical accidents and emerging environmental controversies have caused concern over the consequences of India relaxing environmental and labour laws.

On 1 July a boiler explosion at Neyveli Lignite‰??s thermal power plant at Cuddalore, Tamil Nadu, killed 13 people and injured 17 others. This is after five were killed and three injured in another incident at the same plant in May. Five similar incidents have occurred there between 2014 and July 2020.

A fatal pre-dawn leak of styrene from LG Polymers plant in Visakhapatnam is one of several deadly incidents in recent months

On 30 June, two people died and several were injured after benzimidazole gas leaked at the pharmaceutical manufacturing company Sainor Life Sciences at Visakhapatnam. On 3 June, eight men died and 50 were injured in an explosion at a pesticide factory in Dahej, Gujarat. And on 7 May, a styrene gas leak from a plant run by South Korean firm L G Polymers, also in Visakhapatnam, claimed the lives of at least 12 people while hundreds suffered injuries.

‰??Accidents happen because of routine non-compliance with general and specific conditions with impunity,‰?? says Gopal Krishna, director of Toxicswatch Alliance, an environmental non-profit. ‰??In the name of ‰??ease of doing business‰?? driven by donors of ruling parties, those scientific conditions are being removed ‰?? ignoring lessons from disasters,‰?? he says.

One person was treated for smoke inhalation after a Union Pacific train derailed and caught fire on a bridge over Tempe Town Lake in Arizona, officials said Wednesday.

Plumes of black smoke could be seen rising into the sky beneath a trail of flames on video and images from the site of the derailment, where part of the bridge collapsed.

The train was hauling hazardous material, according to the Tempe Fire Department.

Tempe Police Department Police Chief Sylvia Moir urged residents via Twitter to stay away from the scene, which she described as very dangerous.

‰??There‰??s fire pouring into the lake from the middle of the bridge,‰?? Camille Kimball, who was riding her bike under the structure, told CNN affiliate KTVK/KPHO. ‰??It looks like a scene from hell, truly.‰??

Four persons were injured when a fire broke out in a chemical unit at Chandrapadia village, near Vinjamur, in SPSR Nellore district in the early hours of Wednesday.

The fire extinguishing system in the pharma unit manufacturing pharmaceutical ingredients was immediately activated. The fire, which triggered a blast, was brought under control in about 30 minutes, said District Fire Officer K. Srikanth Reddy, who rushed to the spot.

The sudden increase in pressure while methanol was being loaded into the reactor caused the fire. ‰??'The reactor has not suffered any damage,‰?? the DFO said. The injured were rushed to a private hospital here. Later three of them in a serious condition were shifted to a hospital in Chennai for better treatment.

HANDLING OF AIR-SENSITIVE AND MOISTURE-SENSITIVE REAGENTS IN AN UNDERGRADUATE CHEMISTRY LABORATORY: THE IMPORTANCE OF THE SYRINGE
https://pubs.acs.org/doi/10.1021/acs.jchemed.0c00123
Tags: us_FL, laboratory, discovery, environmental, solvent

A relatively large number of organic and inorganic chemistry reagents are air- or moisture-sensitive, and undergraduate students should be trained in their use. When handling such reagents, it is important to select the correct syringe and to use that syringe correctly. We have found that the best syringe for use in an undergraduate setting is a fully plastic polypropylene (PP) Luer lock syringe. While generally safe, such a syringe should be treated as a single-use syringe. Repeated exposure to an organic solvent causes progressively more resistance to the movement of the plunger, or its complete freeze. Thus, when drawing in a volume of liquid that is close to the capacity of the syringe, the plunger could be inadvertently pulled out causing a spill.
